Minnesota Aging Pathways


Minnesota Aging Pathways, formerly known as the Senior LinkAge Line, is a free, statewide service provided by the Minnesota Board on Aging in partnership with local Area Agencies on Aging. This program connects older Minnesotans, caregivers, and families with trusted resources and support to help navigate the complexities of aging.


Whether you're seeking assistance with Medicare, long-term care planning, prescription drug costs, housing options, or caregiver support, Minnesota Aging Pathways offers personalized guidance to help you live well and age with confidence. Services are available Monday through Friday, 8:00 a.m. to 4:30 p.m., by calling 800-333-2433.

MN River Area Agency on Aging

The Minnesota River Area Agency on Aging (MNRAAA) is a nonprofit organization dedicated to enhancing the quality of life for older adults in southwest Minnesota. Serving 27 counties, MNRAAA provides advocacy, information, resources, and assistance to help seniors maintain the lifestyle of their choice.


Through programs like the Senior LinkAge Line, MNRAAA offers support with Medicare, long-term care planning, caregiver resources, and more. By collaborating with local communities and service providers, MNRAAA strives to create inclusive, age-friendly environments where older adults can thrive.

Wellspring Faith In Action - Watonwan County 


Wellspring Faith in Action in Watonwan County is a community-driven organization dedicated to “Neighbors Helping Neighbors.” Through a network of trained volunteers, Wellspring provides practical support and compassionate aid to seniors and individuals facing challenging life circumstances. From transportation and household assistance to friendly visits and guidance, their volunteers help ensure that every neighbor can live with dignity, independence, and connection. By bridging community care with meaningful service, Wellspring Faith in Action makes it possible for residents to thrive in their own homes and neighborhoods.

VINE - Greater Mankato Area

VINE Faith in Action is a nonprofit organization dedicated to enhancing the quality of life for older adults and individuals facing challenging life circumstances in the Greater Mankato area. Founded in 1995, VINE offers a wide range of services, including transportation, caregiver support, home chores, and wellness programs, all coordinated by a network of dedicated volunteers.


Their mission is to promote independence, dignity, and community engagement, ensuring that every individual has the support they need to thrive. Whether it's delivering a hot meal, providing a ride to a medical appointment, or offering companionship, VINE is committed to making a positive impact in the lives of those they serve.

Interfaith Caregivers - Blue Earth

Interfaith Caregivers – in Faribault County is a volunteer-driven nonprofit organization dedicated to helping older adults in Faribault County maintain their independence and quality of life. Through a network of trained volunteers, they provide essential non-medical services such as transportation, friendly visits, light housekeeping, grocery delivery, and caregiver support.

Health & Human Services of Faribault and Martin Counties

Human Services of Faribault & Martin Counties provides a wide range of programs to help older adults and individuals with disabilities maintain independence, safety, and quality of life. Their services include personal care assistance, home care support, and funding through the Developmental Disabilities Waiver, all designed to help people live comfortably in their own homes.


They also offer adult protection services for vulnerable adults who may face abuse, neglect, or financial exploitation. With offices in Fairmont and Blue Earth, their dedicated team works closely with families and caregivers to ensure that residents receive the guidance, resources, and support they need to thrive.
